As far as I understood the thing you trying to achieve — it's as simple as that: you need to add two inputs to the form like
<input type="date" name="dateA" /><input type="date" name="dateB" />. They will be serialized along with other form values by your existing code. Then you just need to send this array of {name:value} objects to API using jQuery.post() for exampleVB
The coolest way is to add submit button and submit handler to the form element where all magic will happen
